Beauty Habitz collects the personal and health information you provide in consultation and consent forms so it can assess treatment suitability, provide services safely, maintain treatment records, respond to questions or complaints, and meet legitimate business, insurance and legal record-keeping needs.

The information may include your contact details, date of birth, relevant medical history, allergies, medications, treatment preferences, signature and the date and time you submitted the form.

Your consultation information is stored in the Beauty Habitz client portal. Access is restricted to authorised Beauty Habitz staff. Information may be disclosed where reasonably required to professional advisers, insurers, service providers involved in securely operating the system, or where required or authorised by law.

Submitting a treatment form confirms that you have read this collection notice. Treatment consent is recorded separately on the relevant consultation form.
